Nasal pyramid deviation
The nasal pyramid is a structure consisting of nasal bones, triangular and alar cartilages and nasal septum.

The nasal septum supports and helps to define the shape of the cartilaginous portion of the nasal pyramid.

There may be cases where the nasal pyramid deformation may cause functional defects (see picture: humped nose, saddled nose, post-traumatic nose).

Such deformation may cause functional defects or be combined with septum distortions or other nasal pathologies. The recovery of nasal functions as already emphasized, requires correction of both the pyramid and the septum in one operation aiming particularly at reconstruction.

This detail often addresses some sports activities, such as boxing. In these cases the continual trauma of the nasal structure causes various fractures at both nasal bone and cartilage structures with the onset of hematomas resulting in scar tissue. The nasal function is often compromised and surgery foresees the removal of these scars and a precise reconstruction of the nasal structure which is often complicated. Boxers are recommended to undergo this procedure at the end of the career!

To this end, we have to remember that for serious nose injury (where the bone structure is almost completely destroyed and reduced to a mass of small fractures) it is advisable to wait until these small fractures have healed to then intervene. But when there is only one fracture and the treatment has not produced the desired result, it would be better not to wait for the fracture to heal, but immediately intervene with reconstructive rhinoplasty. 

Also this procedure does not make use of pads and is painless. These maneuvers are performed with general anesthesia (without which the patient would suffer great pain) and without the use of pads, makes post-surgical recovery safer and effective with faster healing.

If the entire nasal pyramid is involved, and repositioning the nasal pyramid requires the realignment of all the supporting structures of the nose (septum), it becomes necessary to adopt surgery methods addressing both problems.
